CIVIL COVER SHEET (form # 204) On the Civil Cover Sheet,
page 2, under # 150 (other civil), Mark 163 (other, and
specify TRO.

BLANK MOTION (form # 207) and complete it for the TRO
3. Certificate of Arbitration
(form # 200) Clerk of the Court said form #200, is probably
not needed as this action should fall in the 150-170 area as
you check #163, other and type in TRO.

On the first floor, go to the CIVIL FILING WINDOW. They will
take your original and send you up to see the judge on the 4th
floor to have a judge sign the order. You need to have the
original and 3 copies with you.
In Mesa,
MARICOPA COUNTY SUPERIOR COURT is located at 222 E.
Javelina Ave.
(located So. Of US 60 and West of Mesa Dr.) Call (602)
506-2020 first to verify Judge is there.
NOTE: If
the property ownership (Trustor) is other than an individual(s),
such as a Corporation, Limited Liability Company, etc., you
must be represented by an attorney to obtain the TRO.
