Client Intake form:
In order for this firm to help you
evict the occupant of the property you must provide us with
the following information.

Full name(s) of all occupants.

Social security numbers of the occupants (if you have them).

Address of the property (include city and zip code).

Legal description of the property.

Any particulars you may know about the occupant - what type of
car they drive, number and ages of the occupants, gender and
race.

Describe any efforts you have made to have the occupant vacate
the property (attempted negotiations, offer of payment).

Make sure to let us know if there have been any threats or
attitudes that lead you to believe the occupant(s) will be
aggressive.
Also
provide us with:
A. Copy of the Trustee's Deed or fully executed Lease.
B. Copy of any demand letters to the occupant or responses from the
occupant.
C. In the case of a lease - a copy of the payment history (include dates
due, dates received, dollars paid).
Once we have received this information, along with your
retainer, then we will proceed with the action. You are
to keep this office informed at all times of any changes, or
any attempts to workout the situation. You will receive
a copy of the summons and complaint setting forth the date for
the hearing. You are welcomed to attend the hearing, but
it is not necessary.
Under no circumstances are you to accept any
payments from the tenant. You must contact our office should any such
payments be attempted. If you accept any payments less than what you are
owed, then we will need to cancel the pending hearing. Wait for another
default, re-notice and then start with a new complaint and hearing.
You are to immediately inform this office if the occupant, or
property owner should file a bankruptcy while our FED action
is pending. There will be additional steps that we will
need to take on your behalf once a bankruptcy is filed.
